How can I request therapy or assessment services?

The prospect of seeking support can be overwhelming. We want to make the process of engaging in mental health services is as easy as possible. If you think therapy or an assessment may be helpful for your family, the most efficient way to request services is to complete a New Client Submission Form by clicking here. Alternatively, you can call 904-432-3321 and leave a detailed message on our secure voicemail system, including your name, phone number, and email address. Please be sure to note whether your concerns are of an urgent nature in your message.

Once an intake appointment becomes available, we will reach out to you to schedule a phone consultation during which we will gather some additional information and ensure that we are a good fit for you and your family. If you decide that Nautilus Behavioral Health is a good match for you, you will then be asked to complete new patient paperwork. These forms must be completed prior to your first appointment This will help us better understand the issues your family is dealing with, your preferences for communication, and verify your insurance benefits (if applicable).

At Nautilus Behavioral Health, we will do our best to make the process of requesting mental health services simple, efficient and stress-free.

Should I request therapy or an assessment?

Assessments are conducted to answer a specific question. They occur over a small number of visits and result in a feedback session and written report. If your reason for requesting services is due to a question about your/your child’s thinking or learning (e.g., questions about whether someone has dyslexia, ADHD, a specific learning disorder, a memory problem, giftedness, etc.), you will likely be requesting an assessment. If you need an updated assessment for documentation of your specific learning disorder or ADHD (for school purposes or for your own records), you will likely be requesting an assessment.

Therapy is generally requested when families need ongoing support or intervention to address an area of concern. Therapy may be needed for a very brief duration or on a more longterm/ongoing basis, depending on the nature of the presenting concerns. If your reason for requesting services is due to concerns about emotions, behavior, relationships, overall functioning or performance at school or home, difficulties adjusting to changes, etc, you will likely be requesting therapy.

You do not need to worry if you are unsure of whether you need to request therapy or assessment. All of our mental health services begin with an intake at which we will find out more about your needs and concerns. If you inadvertently requested the “wrong kind of service,” we will address it at the intake and determine what support service best fits your needs.

Do you conduct evaluations for custody?

When a family is in conflict, particularly conflict due to parental separation or divorce, it is very difficult for everyone, particularly for children. In seeking services for your child, please be aware that your child will be the patient. As such, although we may have some responsibility to help to address conflicts between parents/guardians, particularly around issues related to the problem(s) being addressed in therapy, our role will be strictly limited to providing treatment to your child.

We do not conduct custody evaluations or provide recommendations about custody or custody arrangements, including visitation schedules. We will not voluntarily become involved in any legal dispute. Involvement in such matters would hurt your child’s treatment. In addition, your child’s provider is not able to conduct such assessments, given that the nature of their relationship with your child is therapeutic rather than evaluative. Other forensic professionals are more qualified and better able to conduct any necessary evaluations and/or get involved in any legal proceedings.
